Social Security Card:

Pennsylvania Handicap Parking
Click here to obtain a persons with Diability Placard Application Form (MV-145A). This form will need to be certified by a Health Care Provider licensed in Pennsylvania or a contiguous state (New York, New Jersey, Delaware, Maryland, West Virginia or Ohio). A police officer can also certify the request but only when the applicant does not have full use of a leg or both legs, or is legally blind. NOTE: Health Care Providers may only certify disabilities within their scope of practice. Application for Pennsylvania residents requesting a handicapped parking permit — OR — contact the PA Dept of Motor Vehicles.

Click here to obtain a persons with Disability License Place Application Form (Form MV-145). To obtain a disability license plate, you will need to submit Form MV-145 ‘Application for Persons with Disability, Hearing Impaired, Disabled Veteran, or Severely Disabled Veteran Registration Plate or Persons with Disability or Severely Disabled Veteran Motorcycle Plate Decal' (Form MV-145). Like the 'Persons with Disability Parking Placard Application' (Form MV-145A), this form will need to be completed by a Health Care Provider licensed in Pennsylvania or a contiguous state (New York, New Jersey, Delaware, Maryland, West Virginia or Ohio) or a police officer. NOTE: Health Care Providers may only certify disabilities within their scope of practice.

Birth Certificate

Pennsylvania Marriage &/or Divorce Certificates
These certificates are usually obtained from the courthouse in the county where the marriage license was issued or divorce decree was granted. For Greene County marriage/divorce certificates, contact the Greene County Courthouse, Clerk of Courts Office (1st Floor), 10 East high Street, Waynesburg, PA or for questions, call 724-852-5281 between the hours of 8:30 a.m. — 4:30 pm, Monday — Friday.

PA Voter Registration Application Form
Anyone interested in registering to vote may request a Voter Registration Mail Application form through:

Dog License Application Form
Dog licenses are required for all dogs older than three months old. Obtaining a license is a quick and easy process that is essential to the safety of pets. If dogs are lost or stolen, a license can help make sure they get home safely. The fees collected through dog licensing are used to fund dog wardens. Dog License Applications are available at the Treasurer's Office or may be downloaded, completed and mailed with the correct fee to the Treasurer’s Office.
